Asian double eyelid

Surgical options for shape and position of the upper eyelid crease should always be discussed in detail, and final decisions should be communicated clearly well in advance of surgery.
The following classification of crease heights is useful when discussing fold or crease height from the lid margin:
- Low range 2-3 mm
- Medium range 4-7 mm
- High range 8-10 mm
Often Occidental crease sit 7-11 mm above the margin of the lid, but importantly most Asian creases sit lower, usually between 2-5 mm. Patients requesting double-eyelid surgery may be seeking the creation of a crease from scratch or a change in the height of an natural crease.
In expressing a height preference before surgery, it is important to understand that even in the hands of an expert, double-eyelid surgery and subsequent healing are not so precise that your surgeon can guarantee the height of the new crease.
Thus, selecting a "low," "medium," or "high" crease should be viewed as a range!
Poorly-defined creases may be converted into a single dominant crease.
If brow positions are unequal, this will almost certainly impact on the final position of the creases.
The most Natural crease
The most natural-appearing crease on the Asian face is the low tapered. The most unnatural-appearing crease is the high semilunar, with high parallel a close second, both of which will "Westernize" the Asian eyelid. (Note: A high crease cannot be tapered.)
From Most Natural to Least Natural
- Low tapered
- Medium tapered
- Low parallel
- Mum parallel
- High parallel
- High semi lunar
